Through this partnership, clients have the flexibility to self-manage their care package, allowing them to choose their preferred service providers and maximize their care hours.
What is a Home Care Package?
The Australian Government introduced the Home Care Package program to support older Australians in maintaining their independence while living at home. This program offers flexible, personalised assistance tailored to an individual’s healthcare needs. After a formal assessment, funding is allocated to provide services that help individuals:
Maintain wellness and independence through personal care, nursing, and allied health support.
Stay comfortable at home with assistance for cleaning, gardening, home maintenance, and meal support.
Remain connected to the community by providing access to transport and social support services.

What is Aged Care Self Management?
Self-management gives you full control over your Home Care Package, allowing you to make decisions that best suit your lifestyle, care needs, and budget. With self-management, you choose the service providers you trust, decide when and how your care is delivered, and have greater flexibility in how your funds are spent.
Self-management is a way for you to manage the service providers
you use and how your Home Care Package budget is spent. It gives
you the flexibility to:
• Design and plan your own care plan with a dedicated care partner
• Choose the service providers you would like to work with
• Gain more control over how your Home Care Package
funds are spent
• Increase your care hours with lower management fees
Self-management also removes the agency ‘middle-man’ allowing you to reduce your management fees by working directly with your care workers and support service. Thus, allowing you to maximise the number of care hours you can afford through your Home Care Package.
